Root cause analysis (RCA) is a method applied in analysing the adverse and serious events through the identification of problems that increase the likelihood of errors without focusing on individuals’ mistakes.

Finally, root cause analysis (RCA) is a method applied in analyzing the adverse and serious events through the identification of problems that increase the likelihood of errors without focusing on individuals’ mistakes.
RCA is used to identity latent errors (errors within the system) and active errors (errors that occur at human and complex system interface). The practical application of RCA is to aid in preventing all future harm through the latent error elimination that underlie the various adverse effects. SWOT analysis on the other hand helps not only in identifying but also guarding against any unforeseen and pending seen risks to ensure better completion of project and the achievement of a great success rate. Brainstorming is also applied in the postulation and identification of risks scenarios to a project. Thus for the project, brainstorming and SWOT analysis will be applied to a project at all the stages while RCA is mostly applicable after the project implementation; as a way of understanding what is causing problems (Mahmood, Shevtshenko, Karaulova, & Otto, 2018; Oncioiu, 2014; Card, Simsekler, Ward, Clarkson, & Clark, 2014).
Using SWOT analysis and brainstorming techniques, there are a number of strengths and weaknesses. The strengths include strong staff and management buy-in. The main weakness is the time constraint as the project needs to go through regional approval due to the cost. Other weaknesses include inadequately skilled staff, weekend shutdowns to complete installation and system outages. The opportunities include the existing installation of Q-Matic in many labs across the region, collaboration with Q-Matic Corp. The threats include poor clean-up that can lead to safety challenges such as patients’ tripping on cords and selection of another vendor that can take many months. The risks identified under SWOT include late delivery, accidents, and time constraints as a result of the approval process or through the need to select another vendor. Other risks include poor software quality, inadequate designs, scope creep, overly optimistic schedules, contractor failure, base technology not ready for deployment and poor team dynamics.
The legal and ethical issues that may affect the outcome of the project include the privacy concerns. The patient information stored in the system poses a privacy issue which is both legal and ethical concern. When such data and information is accessed in unauthorized manner, the patients can sue to the facility for the data breaches. The potential impact is thus the unauthorized access to the personal identification data that can lead to frauds and other potential damages. The privacy concern will be resolved by ensuring the security of the data as well as the implementation of the access controls where access to the data is granted based on the role of the individual. Further, system security will also be implemented to ensure the data is not accessed in any unauthorized manner.
There are a number of differences and similarities that exist between brainstorming and SWOT analysis in the risk assessment. First, both brainstorming and SWOT analysis focus on addressing the specific questions and problems through the application of free ideas flows. Under SWOT analysis, the ideas focus on the various internal as well as external factors that are instrumental towards finding a solution to the problems. However, there are also some differences that exist between the risk assessment frameworks. SWOT analysis is highly organized and rationalized in such a manner that all the problems and potential solutions can be identified in an effective manner. On the other hand, brainstorming on the other hand is not highly structured as it allows for the free ideas flow as all the data is taken into consideration. SWOT analysis does not focus on the quantity of the ideas that are generated but focus on the facts and data that are internal and external to the project. The other difference is that brainstorming allows all ideas thus it leads to the generation of creative ideas. SWOT on the other hand is less effective for the generation of any creative ideas as it is based highly on facts.
